I think its pretty accurate at 16 weeks. We found out at 17 weeks with DS and it was SUPER obvious that it was a boy so we probably could have seen that it was a boy a week or more earlier. I think whenever there is ambiguity it usually ends up being a girl. My SIL and our friends from church and DH's cousin all had iffy ultrasounds at around 18 weeks and the techs/docs were "pretty sure" it was a girl but not 100%. If you don't have clear penis shot it is most likely a girl. LOL.
