A lot of doctors are moving away from episiotomies and just letting women tear naturally since tearing is usually way less severe.

With Lily I had a small tear that required 3 stitches. I didn't even feel it and I had a completely natural childbirth. You naturally will go numb down there as the baby comes out. The L&D nurse was massaging me as Lily was born, but that is protocol at the hospital I delivered at. I did not do any perineal massage prior to delivery.

From what I have read/researched the position you push in, how relaxed you are when you push, and doing Kegels are more important in lessening the chance of tearing than perineal massage is.
