Try singing Old Macdonald, but in opera-like slow motion, eeeeee-iiiiiiiiiii-eeeeeeeeee-iiiiiiiiiii-ooooooooooo, and get whatever teeth you can during the vowels. Sometimes I tell Daniel to sing la-la-la or eeeee. He has Thomas the Train toothpaste, so sometimes I tell him his teeth are the track and Thomas needs to get in there to "chucka-chucka" around the track. (And of course we do the tooot-tooooot's, too.)
Don't be afraid of the dentist. They may have a suggestion for you, and at the very least, they may offer you some flavored toothpaste. Ours has cookie dough, root beer, bubble gum, pizza ... you name it!
