I'm so sorry to hear about everything you've gone through and are currently going through.

I'm not sure if what you're feeling would be classified as PPD specifically, but from what you are describing, it seems to me like you are definitely going through depression, regardless of what form it's in. Perhaps the pregnancy hormones are playing a role.

Please see a doctor regarding this; I'm sure you know from experience that depression doesn't usually "go away" on it's own, and there are so many resources available. Or perhaps there's some physical thing (for example, hypothyroidism is pretty common, can show up during pregnancy, and can be a cause of depression).

You mentioned you have a supportive fiance, which is wonderful. That would mean that he wouldn't look down on you if you were to look for help.

Again, please see a doctor. I've had PPD, so I know how low depression can make someone feel, and if there is a way you can feel better, it's worth a try.
