Your state's Department of Education should have all of the information you will need. I imagine most states even have the information online as I live in Vermont, one of the smallest/least populated states, and mine had everything I needed on their web site, including all of the forms.

But I know it does varry a lot from state to state. Some, like mine, have pretty strict guidelines and you must submit a detailed curriculum as well as an end-of-the-year assessment done by a state certified educator and portfolio submission, while other states barely regualte homeschooling at all.
