My mom sent me a link to this site to show me the card she is getting for her plastic surgery, and I found this, and I thought I would pass it along!

It can be used for treatments like Clomid, PCOS treatment, IVF, IUI, ICSI, surgery, tubal ligation reversals, and donor eggs.
