Sorry weekend was crazy so just getting around now to posting full update from 1st u/s.

Good news first! RE decided I was 7 weeks even instead of 7w2d, and based on that everything was measuring just fine. Sac was 6w6d, but baby measured 7w2d and very best part of all, we got to hear an awesome hb of 153 which the RE said was really strong for being so early. There is seriously no better sound in the world than a brand new heartbeat

RE puts viability based on that scan as 80% - it's low b/c I have a history of past losses, including 2 that occurred after hb was established...i'm feeling pretty confident because i've never had a loss after a hb that strong, but will feel better after I make it past 8wk scan without issues. I'm officially discharged to OB (yay!) and I'm staying on the progesterone until 11 weeks.

Now for the not so great news...i'm already having some liver issues (really upsetting so early in the pregnancy) which may be related to the Lovenox I have to take throughout the pregnancy + 6 weeks post delivery. But they're not sure...so I have to talk with both OB & peri about it in more detail to see whether a switch to Heparin would make a difference or what additional monitoring will occur. I'm keeping fingers crossed that it's just a medication induced issue and not a sign of larger problems to come

That's (mostly) awesome news, Lara! It sucks about the liver issues. If the heparin doesn't work, will they just take you off it? I hope things get better with the blood thinners. I have my fingers crossed for you!

Laurie - based on what RE said (of course now that i'm discharged to OB he's not the decision maker on this) he thinks it's riskier to remove me from the Lovenox than to continue, but thinks OB + peri should look at whether switching to heparin would/could improve the liver issue. It's a little scary, i won't lie, to see this problem occur so early and basically be told "let the damage continue, monitor and decide if it gets worse what to do". There's a liver specialist involved too (my reg doc called him)...he for the moment seems ok with waiting (i think assuming results don't get worse) but wants to see me & run a bunch of tests once baby is born. It all scares me, but i'm just hoping it doesn't get worse and it turns out to be med induced and reversible.

Why do you have to take the Lovenox for 6 weeks after delivery?

Apparently you're still at high risk for blood clots for up to 6 weeks after delivery (and i'm at a multiplied higher risk than normal pregnant woman now that they know i have Factor II gene mutation & elevated anticardiolipins).

Pregnancy is a pro-clotting state to begin with, and the body tries to stop bleeding post partum from the placental attachment site and the risk of clots is increased.

Lara, I would be frightened too, but I do know that the liver regenerates itself better than any other organ. Chloe had severe liver damage when she was born, but no cirrhosis now (she is a slightly different case because babies are more plastic). I do know that some level of damage should heal itself. Are they watching your fibrinogen and INR?

I am so glad to hear all of Chloe's damage repaired itself (babies are amazing!)...i honestly have no idea what they're watching at this point. The abnormal result initially was just with liver function (came back moderately abnormal) and then reg doc added on a SMA test that also came back mildly abnormal, so no one has mentioned fibrinogen or INR to me yet. I meet with OB's PA on Wed (and i'm hoping to get time with OB herself to discuss this then) and I meet with my reg doc in 2 weeks, and will discuss with her as well. I don't see peri until around 12 weeks, but I plan to make sure he's aware of this too (i figure if it really is something to worry about ONE of these doctors will start running more tests). I really want them to monitor this closely and not just rely on the liver specialist saying he wants to see me post delivery.

I don't know a whole lot about this stuff, just what I learned when Chloe was in the NICU and from her GI specialist. INR is one of the liver function tests, but it and fibrinogen reflect clotting factors, and when Chloe's liver wasn't functioning properly these factors were low, and she wasn't clotting. I was curious if yours were low because you are on blood thinners and how that all interacted, but I don't know that much about it. I was also on Lovenox and they weren't watching anything on me, so I was wondering why they were watching your liver function. Anyway, sorry for throwing out a confusing question, especially since I was just curious and generally know nothing! On a possibly helpful note, Chloe's levels have been up and down a bit, even during her follow up checks (including her creatinine level which has nothing to do with the liver), so I think some fluctuation is probably OK. It's good that they are monitoring you, though. Hopefully your levels stabilize without taking you off the blood thinners.

They really weren't watching...this was good (or bad, i can't decide?) luck that I happened to go in 2 weeks ago for routine bloodwork for annual physical with my regular doctor. I think they run LFT panel routinely as part of that bloodwork, and when it popped a problem they added more tests, including SMA panel. But for that bloodwork, i don't think my OB would run it absent me showing other signs of liver-related issues.
