One thing I *definitely* added to my packing list after being in the hospital is a sleeping mask and earplugs! Otherwise, same basics as y'all have above.

For me:
pjs & comfy daytime outfits (1 each)
socks
brush/makeup(includes gloss/chapstick)/toiletries. Probably my straightener
Hair elastics
Camera + extra batteries and memory card
Electronics & chargers (cell & tablet)
I'm probably going to take a small snack bag for me/visitors etc.
Gifts for the boys from the baby for when they come visit.

For baby Alaina:
my own wipes
3-4 outfits, 1 coming home outfit
couple of blankets
maybe a pacifier, but will probably just use the hospital one if needed.
(And obviously, the car seat and a diaper bag to take the hospital goodies home in. )

For me:
Change of clothes- pj pants, shirt, and nursing bra
shampoo/body wash
toothbrush/toothpaste
deodorant
extra socks
electronics and chargers - kindle, phone, camera
My pillow
Hair ties/hair brush
Gifts for the boys from the baby for when they come visit
Medicine (I only take liquid and it takes forever for the nurses to get it from the hospital pharmacy. Last time they had to crush pills and mix it in pudding - ick!)
Lanolin
Nursing pads
Depends

For baby:
Diapers (I don't want them putting Pampers on my baby since my boys have sensitive skin)
2 Sleepers
Undershirt
Hat
Mittens
Car seat
blanket

Our hospital provides robes, nursing gowns, gripper/slipper socks, and snacks/juice. They also provide blankets, sleep sacks, undershirts, hats, wipes, diapers, etc. for the baby. They list on the registration sheet to bring a lot of stuff you really don't need. This will be baby number three in less than 4 years at the same hospital, so I think I've got it down.

For me :

Extra warm socks( hospital socks leave my feet cold)
My own throw blanket ( it's always too cold for me without it)
My own pillow ( much better than the cheap flat ones they have)
Hair ties
Hair brush
facial wipes
Chapstick
Lotion
Deodorant
Shampoo/cond
Body wash
Pain relieving spray ( the best thing ever to spray on your girl parts!!)
Tucks or some kind of wet toiletry wipe ( reg tp hurts like sand paper)
Camera
Phone charger
Change for vending late night snack run
Snacks for hubby like beef jerky and rock star or star bucks drinks
Big pj pants or sweats to wear home
Few extra granny panties ( I bleed like a stuck pig and nothing feels more gross than wearing undies that have leaks on them)
Comfy bra

Baby:
Outfit for pictures
Swaddle or receiving to go home
Blanket to cover carrier with when going home
Our own choice of binky ( hospital gives out nuk binkies and we prefer the big green monster kind)
A cute sleeper or onesie for visitors
Cute hat for visitors
Our own formula( we use gentle ease and just in case hospital doesn't have it)
Socks

I brought books and baby books and laptops last time thinking I would use them and they sat in my bag. I sleep off and on the first day, eat, have visitors or baby bonding time. There never seemed like a good time for extras. For me anyway.

My list is pretty short, I had a huge list with DD and it was just a bunch of stuff to carry.

For me
-coming home outfit
-nursing bras
-slip on shoes
-Chapstick
-camera with extra batteries
-cell phone with charger
-laptop with charger
-lanolin
-my own pillow

For baby
-coming home outfit and spare
-a couple of light blankets
-diapers/wipes for the ride home (just in case)

My hospital provides toiletries for mom and dad and diapers and shirts for baby.
