They use last period. I have had it explained that when they calculate it from the last period they are adding 2 extra weeks in there to account for the time between the period and actual ovulation/conception times. I also figure it is called and Estimated Due Date not and Exact Due Date for a reason.
Mine was based on early ultrasound (and dates have stayed consistent throughout with ultrasounds). I didn't have AF after DD before conceiving most likely due to a combination of BFing and my PCOS - she was 12 months old when we got pregnant.
