I try to time it from the beginning usually, but honestly if I take a REALLY long time eating I delay a little bit. I heard that especially if you are eating multiple courses like at a restaurant with salad before a main course, then you should time it from the beginning of the bulk of your meal. You could get a salad at a restaurant and then not start your main course for another 1/2 hour, so that wouldn't be fair. My doctor has actually never said anything about it one way or the other. I'm not sure they think about these details...
