I either adjust the hue and saturation to 0 and then tweak, or sometimes I'll split the RGB channels and see which gives the best result. Very rarely do I convert to monochrome.

it depends if Im purposely trying to do b/w I will set it on my camera otherwise I just desaturate in photoshop.

Since i don't have any way to edit yet i usually try to change the setting on my camera otherwise when i go to print it if i want to i will convert it either on the machince or on the program where i'm printing it.

I have two favorite conversion actions that I like to use. One for a more contrasty bright conversion using channels and curves, and another for a warmer conversion using a slight tone. I can share the tone one with anyone that wants it since I got it for free. You can just PM me your email and I'll send it to you. It's a photoshop action and will not work if you have photoshop elements.

I just recently switched to doing it with Gradient Map and then I adjust the contrast and such until I get what I like. They still aren't what I want them to be better, but better then they were for sure.
