My son started preschool and we are required to fill out the waiver form since his vaccines are not up-to-date. We have chosen to selectively vaccinate on a delayed time frame. We live in Michigan. We were handed the state form, which asks us to check the box next to the specific vaccines we object to. Now, IF I use this form I will definitely be crossing out the sentence that says I realize I am puting my child at risk, etc. But my question is: Am I required to identify the specific vaccines I object to? I don't see anywhere in the law where it says I have to do that, yet it's on the form??
