A wedding I coordinated last summer had the Groom's sister stand up as his 'Best Woman'. It has become more popular to have 'bridesmen' and 'groomswomen' included as a part of the wedding party. As far as the attire, men standing up for women can wear a suit the same as the groomsmen, or a similar suit with a tie matching the bridesmaids dresses (if the other guys aren't already). A woman standing up for a man can wear either the same dress as the bridesmaids, or the same style in black, or if you wanted she could wear a fitted suit.
