We do both. The midwives had to fill out an affidavit of live birth and send that to Vital Statistics. Then we had to take Violet to Vital Statistics to sign the papers. We got the BC in the mail today but it's not signed by the deputy for some reason. Lily's is signed, so I'm not sure why Violet's is not. We haven't gotten her SS card yet.
For DS we had to do it, we got a letter from DS pedi appt at 72 hours old and a letter from the midwives and had to go downtown to the vital records office to apply for his BC- we got it that day. Another day we went to get his SS- with his BC and the letters. It was a pain in the butt because they were giving us fits he didn't have an immunization record. stupid laws.
this time around will be different....since we're living overseas. I believe our MW fills out the paperwork and we get a birth record sheet or something, then we'll have to go to the US embassy to get his/her SS and passport. not looking forward to doing that with a newbie.
