I did not write a letter per se, but i did write out a long-ish birth plan which contained what i wanted for my birth and also what I wanted in the event a C section couldn't be avoided. I will have to go back and look for it, but If I find it I'll definately post it for you.
As for the VBAC...what I did was just make it clear from the outset of my pregnancy following the C section that i would not be consenting to a repeat section just because. I even went so far (and so immature) as to mark a giant sad face on the words C section on my chart under previous birth history. The OB did a double take at it but smiled. I was very forthright from the beginning that I expected a vaginal birth. I was lucky that I never encountered anything but support from the OB and the CNM....
