PSE:
First off, do NOT edit your levels and do not go to photo styles to create black and white!
Open your picture then go to Enhance>Convert to Black and White

You'll see the original a preview, 6 different choices, and 4 sliders. Just leave the sliders alone for now. Go through the 6 choices and pick the one that gets the skin tone to the type of level you want. In my picture I'm wanting the skin to be almost pure white.

Next go to Enhance>Adjust Lighting>Levels (or ctrl+L/command+L)

The first step we'll do is to move the left slider in to the right making the shadows darker. Move it in until your darkest parts are BLACK

Next move in your middle slider to the left until you get the bright spots LIGHT GRAY.

Last, move your right slider to the left until the highlights are WHITE. (I'm using the skin as the basis)

To finish go about your normal sharpening and finishing editing.
