Hi My name is Kary 34 dh Tom 34 also... a little info on us. We have been together since 1993. Married in 1998. Started talking about having kids in 2000... 2001 DH Diagnosed with Multiple Sclerosis. TTC was put off so I could go back to school to have a career that could support a family. So fast forward to May 2008. I finally graduated after 6 years of part time schooling and working full time while also trying to raise my teenage son (will be 19 next month).... I went off my pills the day I passed my boards in July. The journey began that day. Over the past 15 months.... we have had three early losses. After the last one last month we went to see an RE. Blood work.. for the most part was normal.. I have the MTHFR mutation.. but the secondary test came back ok... so they are not sure if that is an issue or not. So on to the SA.. Got the phone call today... the Dr is surprised we have conceived 3x's. The numbers are... (these are the only numbers DH wrote down)

12 million- should be > than 20 million
13% motility should be more than 50%
3% morphology they like to see above 4% (I am confused about that one)

These are great numbers are they?? And What about the morphology?? Isn't a lower number better? Can anyone explain this to me?

Welcome! ((hugs)) I'm sorry about your losses.

To answer your questions..

The count is low - do you know if that is the total count or the count per mL (concentrated count)? Either way it's low.

The motility is low as well - they want at least 50% to be motile.

Morphology always confuses me because different Doctors do SA's differently - with each one that we did - they wanted at least 30% NORMAL morphology - so that one I don't have any answers for.

I think it's a really good sign that ya'll have conceived 3 times on your own already - that's amazing!

I always recommend getting a second SA done - because they can vary widely. ((hugs))

I don't know much about numbers since my DH didn't have any spermies to compare your numbers to. I do recommend another S/A and also is your DH working with a urologist?

I think I've seen your posts on some of the other areas, but just to fill you in on us a bit, since we had an "abnormal" SA as well. I've been off the pill since May 08, no pregnancies, got some prelim. bloodwork done in Sept which showed my progesterone was low, doc started me on Clomid (no luck with 1 round). Before going on to another round I made DH do an SA just a few wks ago. I did A LOT of reading, research, trying to learn as much as possible because I cried & cried when my doc called to say his was "slightly abnormal'. Now that I've had time to process & digest that, I'm a little more chilled out. Here was his numbers just for comparison:
NORMAL:
sperm count per mil: 50
motility 51%

ABNORMAL:
viability 54% (normal >75%)
morphology 26% (normal > 30%)

Now as far as the confusion about morphology I pulled up a few articles online explaining the confusion about it; there are 2 scales that can be used. Kruger introduced the stric Tygerberg criteria & thats divided into 3 categories: normal morphology group (>14% normal forms), good prognosis group (4-14% normal forms) and the poor prognosis group (<4% normal forms). The WHO (world health org) however uses a completely different scale. The cut off value changed from 50% to 30% for them. I will try & find this link of a really good article I found explaining alot about abnormal morphology if you are interested...just let me know. Anyways, I have thought about having DH repeat his SA again in about 6-8wks. I dont know if my OB is giving up on us, when she had called to tell me the results of his SA she said she'd give me 2 more cycles of Clomid to try but then the RE she talked to said "we'd be good candidates for IUI" ...the guy doesnt even know us, so I freaked a bit. Going to wait it out & see what happens a little bit longer. Sorry if this was really long & drawn out but hopefully it helped a tad

Welcome to this board! I'm sorry for your 3 losses. I'm surprised you gotten pregnant 3 times with that kind of SA result. That means HOPE for you!!!

My DH's SA was 8.2 mil and NO motility ... doc recommended him do 2nd SA.. which he has not done so far. He will eventually. We were told if 2nd SA shows the similar results, then we would have to do IVF/ICSI. We have been TTC #1 for 3 years with NOTHING.

You do have hope since you got pregnant 3 times but have the doctor check his chromosomes as there could be abnormal sperms or chromosomes that cause miscarriages?

He had bloodwork (hormone, chromosomal, VD) .. that has come back all normal so far.. I just don't know.... I don't know, were to go from here.

Hi! Welcome to the board! I don't know about the numbers either since my DH had 0 sperm, but I hope a 2nd s/a will turn out better, especially since you've gotten pg 3 times. Good luck!!
