Our favorite place is a local bar and grill called Mr Bills. BF loves their cheeseballs and tenderloins. BF is a delivery driver so we know exactly how much you have to rely on tips. We always tip unless someone is truly worthless. $5 is our max.
My favorite place to eat is this little Mom and Pop place here called "Little Italy" aka Mamma Rosas. I always go into a resturant with the full intention of tipping but I have to admit I have been guilty of NOT tipping. Dave and I have been out to eat where literally the waiter would take our order and not do anything else. I feel like it is their job to serve so they should do it. I hope that does not seem harsh!

I love Olive Garden. I always tip though but I've never really had bad service, although we've had waiters/waitresses that put HOT food in front of Ash when she was younger and touched everything. We moved it before she touched it.
We don't really get to eat out too much just because of how rural we live.... There is an area in the City about 2 hours from here that has some amazing restaurants. In the summer they all have patios, and any one of them I'd consider a favorite.
I generally tip a min. of 15-20% sometimes more for great service and sometimes less for poor service. If I feel that the service was really poor, I will not tip. Unless we are in the US. Here, servers still receive Min. Wage ($10/hr) in addition to tips, so they aren't expected to survive on the tips they earn. I know in many areas of the states though, a server may only earn a buck or two an hour and really counts on those tips to get by... I would feel really awful not leaving something... just not the full 15-20% That said I would expect that when you are counting on tips for good service the service should be pretty good!
