Help! Need info about Midwife abilities for newborn blood testing
 
We are still desperately trying to choose which birthing option we are going to be able to pick. Half of our decision is what insurance will cover, which is sad but financially a reality. The other part is knowing if a midwife can easily test at a homebirth or birth center birth for antibodies, through coombs or blood cord testing. I have Anti-M antibodies which can be absolutely nothing, or can require some treatment for jaundice, or it can be as severe as a newborn blood transfusion, if we didn't need an intrauterine blood transfusion.

Anyways, we won't have a clue how serious it will be until near birth. My blood titers could stay relatively normal and nothing extra will be required, other than a quick check at birth and all is clear, or those final days could show that we are looking at newborn anemia or jaundice and higher levels of care.

I have a good hospital picked if we are forced to choose the hospital option. I'd still rather choose home birth or birth center if possible and I'm not technically high risk at this point. I am also trying to choose a new care provider and am lost on whether or not I should pursue a midwife group who can do HB and BC births knowing that I could be forced into a hospital birth later. Can midwives do the cord blood or coombs test? Or will we need to leave the birth center a few hours later and go to the doc just to run that stuff? I SO need someone to help me narrow this down because I just go in circles

Can a midwife easily do those kinds of tests right at birth? Sorry this is so rambly, it's just all the stuff swimming in my head and I need to choose soon since I am already 17 weeks.

Hi!

I think if you go with the midwife option, you could maybe find one who has privileges at a hospital so if you realize you need the hospital at some point she could go with you.

I think what you're asking can be done at home. I have a blood type that leads to jaundice as well that I did not know about till my son had it. Apparently my blood type and my sons blood type mixed together were not good and he developed a bad case of jaundice and we did all of his blood testing at home. The nurse came to our house and did his pricks there. The birth center sent her. I would ask in advance. I didn't know to look for this though. Our birth center sends a nurse to the home at 2 days pp to every one and they to do the blood pricks for every one too as well and send them off to the lab. Our pedi was able to get those lab results by the serial number. Again, ask your midwife or bc rep ahead of time and they should know. Ours was very informative of every question we had and there was never any unknowns.

My midwife draws cord blood immediately after the cord is cut and takes it to the lab right away. The lab gets back to us within hours with pertinent information about baby's blood type.

I would ask the midwife or birth center you are considering what her policy and available services are.

I have RH- blood which required a cord blood sample and a blood draw from me right after birth to see if our blood had mixed and whether I would need a rhogam shot. My midwife was able to do it all during my home birth. We had to play the juggling game with our insurance company too. They won't cover midwives at all so any labs she ordered would be denied. My family doc was totally on board with my plan for a home birth so he wrote the orders for all the blood work I would need so I could get it covered. I gave them to my midwife to keep in my chart. She dropped the samples and orders off at the hospital when she left my house after birth and that was that. I am so thankfull my doctor was willing to help with that because the labwork and rhogam would have run me around $700 out of pocket on top of paying for the birth.

Most midwives will schedule you for a consult where you get to ask questions and get to know eachother before you hire them. I would ask then how they would handle it.

I would interview a few midwives and ask each. Home birth midwives all practice a bit differently.
