Joining ~ Recurrent Pregnancy Loss, Diminished Ovarian Reserve, and Possible IVF

Hi Ladies! I'm Lindsey (31) and DH is Chad (34) and we've been trying for a baby since April 2011 and have been pregnant 3 times since (sadly lost them all). Took us 12 cycles the first time and I was pregnant for nearly 22 weeks and lost baby last June 2012 due to a chronic placental abruption but baby was otherwise healthy. 2nd pregnancy (Nov. 2012) we got pregnant on the 4th cycle TTC and that one lasted 5 weeks and ended up needing the methotrexate shot to complete the m/c. 3rd pregnancy was a chemical about 3 months ago.

Tests I've had done: Thrombophillia Panel (last year after I lost Cade at 22weeks) everything was fine. I had a pelvic u/s done after that pregnancy to make sure that I didn't have fibroids in the uterus and that was fine. I do, however, have fibroids on outside of the uterus but was told those wont hurt fertility. After the next 2 losses, decided it was time to see an RE to make sure nothing was causing the losses.

Tests with RE (started tests last week on 7/20):
~ On 7/20, had the CD3 b/w and u/s. Found only 2 antral follicles on right ovary and none on the left :( and an FSH of 9.4. Doctor had me start BCP's this day when he called with my results telling me that I had low ovarian reserve so we will need to get me through the testing as fast as possible and get me pregnant ASAP. Spent this entire day/night devastated feeling like it was the end of the world and that we may never have any biological children.
~ on 7/22, had the physical exam and vag cultures, everything was fine. Doctor stressed again this day about my low ovarian reserve. I asked him what the solution to this would be. And he told me we need to "get the eggs out and pick out the good ones." He already has me scheduled to start treatment on Aug. 14th but will need to finish testing and have our consult with him to discuss everything in detail.
~ HSG ~ Tubes were open
~ Sonohystergram found a polyp in my uterus. I will need surgery to have it removed next week.
~ Also had more clotting disorder b/w done that my OB from last year didn't include in the testing he had me do and I'm still waiting on those test results.

So basically, we still are waiting for my surgery to happen to remove the polyp before we discuss what treatment is going to be but it's sounding like IVF is what my doctor is thinking.

I really hope my doctor has a lot of information ready for me at the consult b/c the thought of IVF prior to this testing wasn't even a thought since we have gotten pregnant on our own 3 times. But now I am really really nervous about the extremely low count of antral follicles and the elevated FSH, especially for my age. I'm also going to ask about the uterine polyp and if that could have been a reason for the losses?

Now that I know about the problem of my eggs diminishing I'm really feeling really nervous like I'm running out of time... And that a decision regarding whether or not to do IVF is something we need to make ASAP. So I'm wondering what your thoughts on this might be?

Thanks for your help ladies!

Hi moonangel! Welcome. Im happy to see you though im sorry of whats made us meet again.

Some of the ladies have ideas on helping out fsh. I'd recommend getting fsh and follicle count repeated. It's possible what is found is true, but it is possible for things to vary a little. Also did DH get a semen analysis? Both partners should always be evaluated

I'm sorry about your news. Just know that there's hope. You have some time. Best of luck. Lots of hugs.

Hi Sunrise. Yes, I forgot to add that he did get that done just Friday when I got the sonohystergram done. We don't know those results either. So when we have the consultation with the doc hopefully next week he will have all the results for us. We also had a ton of fasting b/w done and he said they were "Good" and will give us all the results when we meet.

welcome and good luck. Have they done an AMH level on you? What was the rest of your CD3 bloodwork like? We have several ladies on the board that have dealt with high FSH/DOR and can probably give you some advice.

Re: Joining ~ Recurrent Pregnancy Loss, Diminished Ovarian Reserve, and Possible IVF

Hello, thank you!

All they have told me so far is the following: (I'm hoping to get all results at our consult which will hopefully be next week):
Low AFC @ 2 (At the u/s I heard the tech say the right had 2 and left she didn't say anything so I'm assuming those are the 2 on the right ovary. Does that mean my left ovary just isn't even working at all?). They like this # to be around 12.
FSH 9.4 (They told me they like to see it below 8)
LH 3.3
Progesterone 0.3
Estradiol 33

I've done a lot of research on this and I know now the AMH is really important but they didn't tell me that. I will be asking about that for sure at the appt. I've also read online that IVF isn't recommended if you have below 4 AFC's..so i will ask about this as well if IVF is his recommendation.

Hi honey...I recognize you from the TTCAL board ((HUGS))....Can I ask how old you are?

My clinic looks for an FSH of under 10, so I do think you might still be in the normal to high range (mine was 12 a few months ago and I just turned 37) .... my first IVF I only got 4 eggs (and we had our daughter), but this time I got 11 (transferred 2 and had 2 to freeze). I know your AFC is low, but it can vary from month to month, this time for me, I think my AFC was only like 6 or 7, but I ended up with 11 eggs, so some can be hiding or pop up later....

The good news is all your other tests seem to be good (except that pesky polyp)...

I'm sure Lash will have much more to add.

If you have any IVF questions ask away - or join the thread on the IVF subforum. I did IVF in 2010 and just did a cycle in March/April...and I'm currently in a frozen embryo transfer cycle...since I m/c in early June.

I know this is not the news you wanted to hear re IVF path..but we are here for you ((HUGS))

:welcome:
Wow you have been through a lot :( I am so so sorry!

So I am new to IVF and learning myself :)
I had a a bad lab done that showed FSH 9.7 and my IVF RE wasn't even fazed by it. He said as long as it was under 10 he was fine. I am not sure what that means with your low AFC tho and am totally interested in what your AMH is.
I do feel you can help your numbers tho. When i thought I had high FSH I got on a bunch of supplements because I read so much about how helpful they are. Lash can help with this... These are what i am on: IVF or bust!: Vitamins & Supplements and since we have MF issues I have had DH on a bunch too and we have definitely seen a difference at least with his count! Dh's: IVF or bust!: DH's info

I had a SIS and they found a polyp like you did. I just had surgery on Friday to remove it. It was pretty easy thankfully. From what i have read YES a polyp can cause m/c. Have they told you how large yours is?

I think since you have gotten successfully PG there is a lot of hope. I am sure this is a lot to digest! But you sound like you are ready to take this on, research and advocate and that's half the battle!

I'm sorry about your losses and can't offer much for all the testing, but I'm going to share my thoughts..in hopes it may help.

My husband and I have been ttc for over 4 years, and it was a long and rough journey. We tried so many different supplements, medicine, positions, and anything else I read that might work. TTC was really taking a toll on me, and I had read that fertility drops at 35, and we decided to finally see a specialist. The specialist believed the problem was with my tubes because our tests came back good. He told me that we could try some different meds, but he didn't think they would help and recommended IVF. I was scared and unfamiliar with IVF, I always thought that was "my last hope" and part of me didn't want to admit that. Anyways, we went for it. It was nothing like I expected and was very easy for me. My 1st transfer was successful, and we are now expecting twins. I wish I had done IVF years ago instead of torturing myself every month. This is just my opinion, but if your insurance covers IVF, I would really look into that and possibly save yourself a lot of heart ache. Good luck to you!!

Melissaleigh ~ thank you so much! :)

Lucy S. ~ Thank you so much for that information! I'm definitely going to start spending some time reading and researching for sure. He said the polyp is 1.5cm. He didn't seem to think it was big and said the surgery would be quick and easy. And that makes me feel better that you are also saying it's not very bad. :) Now I'm ready to just get it over with! But probably won't be until next week.

smsturner ~ Hi susan! Thank you for stopping by and saying hi! :) I can't believe you are 10 weeks already! That is so awesome!!!

Jes81878 ~ thank you so much for sharing your story with me! I can't even imagine. 4 years is such a long time to battle and trying for a baby. I totally get where you are coming from. And congratulations on the twins! Such happy news!! :)

Chichica ~ Thank you! I appreciate it. :)

Re: Joining ~ Recurrent Pregnancy Loss, Diminished Ovarian Reserve, and Possible IVF

So the really hard part of all of this is that we get pregnant. I know that sounds odd, but it's the part that makes you keep going and going. It kept pushing us forward, and then when we got pregnant naturally last year, it was just unfathomable to not keep trying with our own eggs, tests and hormones be ******!

It's very much a toss up to be really honest. You'll get a ton of victory stories and then you'll hear of plenty that moved on to donor eggs. You are in a precarious situation in that IVF will be much less likely to work although not impossible. There are multiple views. One is that we'll likely make very few eggs in IVF, so it's best to go do a minimal stimulation cycle and only attempt to get 3-4 good quality eggs. We face both a quality AND quantity issue. We don't have many left and the quality isn't so great either. But there are still some good ones in there.

The question is how far you go and how long you wait to find the good one. I also couldn't financially keep doing IVF when it might take the 9th cycle to get the actual good egg that made a baby. You might get lucky the first time. We did IVF and then got pregnant completely naturally, so that is always an option.

Plenty of women like us also go back to more simple methods like Femara. Since we wont' get many eggs anyways, why not try boosting ovulation methods instead of the super high tech IVF approach. It's something to consider. Since I did get pregnant, my RE was willing to do Femara and IUI.

I on the other hand just cannot wait anymore to find my own good egg, which is REALLY hard after having a baby naturally. Loss makes the waiting impossible though, which puts me into trying donor embryo's instead of using my own eggs.

The main thing you need to hear is that there are options. There are vitamins to reduce FSH. Supplements to enhance egg quality. Multiple options and approaches to IVF. And a few that are a few steps back like clomid. There is egg donation, embryo donation, adoption. There are options. But it will require some grieving at each stage and deciding if your final goal is to have your baby, your genetically related baby, or to be a parent in general. That answer is different for each of us.

Let me also link you to network54 which is all high FSH and low ovarian reserve women
New High FSH Support Forum

My diagnosis was a few years ago. FSH at 33 and my egg counts were super super low. We got retrieved 7 at the final day of IVF but it didn't work in the end.

Also, processing all of this after stillbirth is really really hard. It starts to feel like the Universe hates you doesn't it!

Do you go to any meetings for stillbirth or infertility?

Re: Joining ~ Recurrent Pregnancy Loss, Diminished Ovarian Reserve, and Possible IVF

Thank you so much for all of that info, Lash! It is very helpful and has helped me with research as well. I'm so sorry for all that you have gone through. :( I hope and pray your rainbow baby is right around the corner!

Today we had our 1st consult with the doctor to finally go over all of the test results officially and discuss treatment plans. I found out that my Vitamin D level is super low so I need to go on a supplement for that (he said it was 14 and should be over 30). They will recheck that in 3 weeks.

FINALLY found out my AMH and it is 1.28 ng/ml. My doc said that is on the low side. I need to do research to find out just how bad that is.

My doc said we have 2 options: IUI with FSH or IVF. Next week on Tuesday after my Pre-Op appt we will be meeting with the lady to discuss pricing.

I also asked my doctor about natural ways to help out the eggs and he recommended CoQ-10, he said it has proven results. So I'm going to start that supplement.

My surgery is set for next Wednesday to remove the polyp. He said it is possibly the polyp caused the placental abruption of my 22 week loss if the placenta was on top of it or next to it. So it's a good thing we are getting that out of there. He still thinks the 2 earlier losses were b/c of a bad egg.

He also said it is possible that some months I could have more than the 2 follicles on one side and I asked him that if he thinks my left ovary (Has 0 follicles) has officially retired. He said he's not completely sure and that it could still have more eggs in it. He will know for sure if it responds to medication. Also, it's possible that I have endometriosis on the ovaries that has caused damage.

So, for now, I will be taking the supplements and just continuing on to next week for the surgery and more info on IUI/IVF. Also, on the 27th, I will have a nutrition class I will be going to.

Also, I'm going to be reading the book Infertility Cure. I heard good things about it online!

I don't know much about IUI with FSH. So I have lots of research to do in case that is an option we decide to try out. Does anybody here have experience with that?

BTW...all of my husband's test results came back perfect! I'm SO jealous! :)

Lash August 15th, 2013 01:22 PM

Honestly that AMH doesnt make me bat an eye. Mine is .29 and most high FSH ladies discover theirs is under 1, usually under .8
