Can you get the midwives to itemize so that the fee is less since you transferred into their care so late in pregnancy?

Our midwifery group is also considered out of network with my insurance. BCBS is anti-home birth, so they won't consider any home birth midwives in network (at least here). I have to pay a higher out of network deductible and 40%. My midwives told me that when they used to submit the claim as a "global" claim for the whole pregnancy and delivery, they would get a statement back from BCBS that they would only cover about $2000 (and then the mom would get still get stuck with 40% of that plus the difference between the $2000 and what the group actually charged), but once they started submitting the claims with each visit, test, delivery, post-partum care, etc. itemized separately, BCBS started paying for more. For me, BCBS agreed that the midwives would get paid $4500 (what the midwives actual fee is for me), and I have to pay 40% of that plus my deductible.
