In Illinois we have to submit proof of pregnancy, two letters from non family members saying they witnessed a live birth (this will be difficult for me), and I will have to write letters of refusal for all sorts of testing. Here they can't force you to test for anything (Illinois is relatively generous about 14th amendment rights) but they can frown a whole bunch. There may be other requirements, but I forget what they are.
