is it always automatically all of the foreskin or do doctors leave some behind now? I've heard stories about 'loose' circumcisions that sometimes end up having problems

It probably varies by Dr. You'll have to talk to yours. It's not something I plan on ever doing so I don't know a whole lot.

I'm pretty sure its all the foreskin. we had it done for both my boys and it went ok with both.

I'm not sure how it's supposed to look after on a baby but I had the plastibell done and it healed completely in 4 days and it seems perfectly normal with no issues.

Loose circs are better than taking all the foreskin, because you don't know how big baby will be. The more foreskin they cut off they more likely they will have a tight foreskin and painful erections. And with a 'loose circ' the boy can restore his foreskin easier if he decides he'd rather be intact (only restores the function and not all the feeling).
