I have two that come to mind. I used to make my Oma's biscuit recipe all the time. I had a friend over and I had told her how good they are and so I made them for her. They came out inedible. I mean, really inedible. They were soooooooooooo salty. The next day I realized what I had done wrong. I used baking soda instead of baking powder. Huge difference for saltiness. They all had to be thrown away.

The other mistake...I was making brownies to bring to a friend's house. I always seem to make mistakes when there is an audience or I'm supposed to be bringing a certain dish somewhere. Well, anyway. I was making the brownies and I was in a hurry. I forgot the eggs. I realized while they were cooking, but by then it was too late to add them. They had to cook for like an extra 45 minutes to even be cooked. And they didn't raise at all. And they were sooo impossible to get out of the pan. The taste was actually fine, but the texture was completely wrong.

When I was about 8 I was getting my first chance to make dinner with minimal help. My mom had me make chili - something pretty easy. Well, I thought the chili needed a little more spice so I looked in my mom's spices and saw one saying "All Spice" I thought surely that would make it spicy, so I put a ton in. Yeah... if you know anything about all spice you know that spicy it is not. Talk about a seriously misnamed spice. My family still picks on me about that.
